Currently, our office is accepting the following vision plans:

Once your appointment has been scheduled, we will confirm that your specific plan is in-network with our office prior to your appointment.

All exams not using the insurances listed above will be self-pay. Our office also accepts HSA and FSA cards for payment. An itemized receipt can be provided if we do not accept your insurance plan or if your plan is out-of-network with our office for out-of-network reimbursement from your insurance provider. Please confirm with your insurance the amount they will reimburse.

Self-Pay

For patients not using insurance:

Eye Exam (comprehensive eye exam that includes a glasses prescription) - $99

Eye Exam with Single Vision Soft Contacts (comprehensive eye exam that includes a glasses and contact lens prescription) - $159

Eye Exam with Monovision/Multifocal Soft Contacts (comprehensive eye exam that includes a glasses and contact lens prescription) - $179

Eye Exam w/ Single Vision Rigid Gas Permeable Contacts (comprehensive eye exam that includes a glasses and contact lens prescription) - $199 for new patients or $159 for existing patients to our office

Eye Exam w/ Monovision/Multifocal Rigid Gas Permeable Contacts (comprehensive eye exam that includes a glasses and contact lens prescription) - $199 for new and existing patients to our office

Optomap Retinal Imaging - $39
